RESUMO

Archive documents and sources in scientific electronic databases were analyzed in order to study the historical origins of the health resort business in terms of children's health resort medicine in the Samara government. It has been established that the period of origin and intensive development of balneology for children in the Samara region occurred at the beginning of the 20th century and the first children's sanatoriums began to function thanks to the initiative of the Society of Governmental Physicians for charitable funds of the Samara nobility and merchantry in territories with natural and climatic therapeutic factors, namely Sernovodsky resort, Barboshina glade and Postnikov ravine. A comparative analysis of the dynamics of morbidity structure of children admitted for sanatorium treatment, as well as general education institutions, from which information on the health of school pupils is received, has been conducted over a period of 120 years. Despite the change of years and generations, there is a general concept of sanatorium health improvement of school pupils as a future social and economic pillar of society, with the formation of a healthy generation for both the region and the country in general.

RESUMO

OBJECTIVE: To determine the characteristics of sanatorium-resort therapy impact on children with post-COVID-19 syndrome of various severity, as well as to reveal association of its severity with family history data and genetic polymorphisms of alpha-1-antitrypsin-serpin-1 complex. MATERIAL AND METHODS: This 2-week retrospective cohort study involved 42 adolescents after new coronavirus infection (COVID-19). The first group included 28 (67%) patients (mean age 13.1±0.8 years) after mild COVID-19 (without confirmed coronavirus pneumonia), the second group - 14 (33%) patients (mean age 14.5±0.1.2 years) after moderate or severe disease (with confirmed coronavirus pneumonia). A complex of procedures, according to the approved standard, was prescribed for all patients admitted after outpatient and hospital treatment to the pulmonology department of the state children's sanatorium in order to aftercare. The certain follow-up parameters were evaluated: symptoms severity, life quality, respiratory function and respiratory gases, as well as family medical history and alpha-1-antitrypsin-serpin-1 complex. RESULTS: Patients after moderate and severe COVID-19 had initially lower and less dynamic growth of integral life quality index, more torpid follow-up rates of spirometry, pulse oximetry and exhaled gases. Additionally, the higher incidence degree of adverse family medical history associated with respiratory illnesses was established in the group after new coronavirus infection. Moreover, relatively more deficient alpha-1-antitrypsin and more frequent heterozygous polymorphism type of serpin-1 were found in the group after severe new coronavirus infection. CONCLUSION: The revealed complex of epigenetic and genetic factors may indicate various risk and development phenotypes of both acute and chronic respiratory diseases.

RESUMO

Health care in general and medicine in particular play an important role in the geopolitical landscape and the political positioning of the state in the modern world. The health of the citizens of the country is the most important resource of national security. This article, based on a SWOT-analysis, highlights the strengths and weaknesses of the foreign and national resort industry as a part of medical diplomacy, with decomposition to its individual participants. The undoubted advantage of our country in terms of humanitarian policy, on the world stage is shown, specifically in the context of national key success factors, including the technological capabilities of domestic medical science and practice, regarding the availability of trained staff, specialized variable climatic sanatorium and resort institutions network with unique technologies and natural healing resources, coupled with international experience in humanitarian cooperation, developed healthcare system and sanitary and epidemiological supervision of the country. Medical diplomacy and national resort medicine as an active participant of the branch, are strategically important areas in public diplomacy, having the ability to play an important role in achieving national goals in geopolitics.

A total of 71 patients with bronchial asthma (BA) were examined and treated based at a specialized health facility of whom 43 ones presented with mild asthma and 26 patients with moderately severe asthma in remission. The median age of the patients was 12,04 ± 2,08 years. Spirometry parameters: MOS50, MOS75, and PSV, proved to be of high informative value combined with high enough sensitivity and specificity. The significant difference between the groups as a whole (p < 0.05) were documented based on the serum leptin levels that were higher in the overweight patients and especially in the girls (p < 0.05). The leptin level was a similarly sensitive/specific (AUROC > 0.5) and informative (SCDFC > 0.5) parameter which confirms its diagnostic value given it is employed in combination with overweight for the evaluation of the status of the patients suffering from asthma. Negative linear and non-parametric correlation between the functional parameters and the serum leptin levels in the overweight patients with bronchial asthma suggests that the enhanced leptin concentration may serve as a marker of strong obstruction and severe inflammation at all levels of the respiratory tract in the patients presenting with a combination of the two conditions.

RESUMO

Respiratory function studies play an important role in diagnosis of chronic dust bronchitis, both for the diagnosis verification and for determining the disease advance. The study is characterized by step-by-step assessment of respiratory function with body plethysmogram, flow/volume chart and bronchial spasmolytic test. Discrimination and regression analysis revealed leading functional parameters in chronic dust bronchitis: respiratory pathways resistance, pulmonary residual volume, bronchial spasmolytic test, forced vital lungs capacity, peak output flow.

RESUMO

Occupational bronchial asthma remains rather prevalent condition developing due to industrial allergens both of organic and inorganic origin. Diagnostic tool is a complex approach including study of occupational and allergologic anamnesis, immune and allergy status, respiratory volumes. Diagnosis leads to first therapeutic step--elimination of contact with occupational allergen. Treatment of occupational asthma includes inhalation steroids, beta 2-agonists, cholinergic antagonists, methylxanthines. Objective of this treatment is maximal control of the disease with minimal therapy facilities--these are contemporary ideas of medical management for such occupational patients.

RESUMO

A unified scheme for comprehensive interview of workers of a large metallurgic works who come in contact with occupational noxious agents that cause respiratory organs was used to select 300 workers (199 men and 101 women). They underwent a bicycle exercise test that involved the determination of forced vital capacity (FVC), forced expiratory volume within the first second (FEV1), peak expiratory air flow (PEAF), and the Tiffeneau index. Moreover, the flow/volume ratio was examined on a Flowscreen device (Erich Eger) and a test with a bronchodilator (ventoline, 200 mu/kg) was also made. The authors conducted 64 assays of eosinophilic cationic protein (ECP) and 64 assays of mucinic antigen (MA). Serum ECP and MA levels were measured by immunofluoresence and sandwich-immunofluorescence, respectively. Screening functional indices obtained from the flow/volume and bronchodilator tests are sensitive in both manifestative and latent dust-induced bronchitis so they permit identification of latent and obvious airways obstruction. ECP is a highly reliable diagnostic criterion for the asthmatic form of chronic occupational bronchitis and MA is for its inflammatory form.
